SURVEY CONSENT


Who is conducting this survey?

  • This survey is conducted by Trans Care BC, a program of the Provincial Health Services Authority (PHSA). We coordinate and monitor gender-affirming care, including surgeries, in BC.

Why are you conducting the survey?
  • We want to learn about people's experiences with the services provided by Trans Care BC’s Health Navigator team. This information will help us improve services for people in the future.

What am I being asked to do?
  • You would be asked 8 questions about your experience and background. You could skip any question you did not want to answer. It would take less than 5 minutes to complete the survey.

Do I have to complete the survey?
  • No. Participation is voluntary. There are no consequences if you choose not to participate.

What will you do with my responses?
  • The survey administrator at Trans Care BC will share a summary of survey results with the Health Navigator team and supervisor of the team. Individual survey responses will not be provided.

What kind of personal information will I be asked to provide?
  • You will be asked to share your gender and when you last interacted with the Health Navigator team. You can provide more information in comment boxes. You should only tell us things that you are comfortable having us share with your the Health Navigator team and supervisor of the team. You also have the option of giving us your contact information so that we can follow-up with you. We will not share your personal contact information outside of Trans Care BC.

How will you protect my personal information?
  • Your personal information is protected by our privacy law in BC called the Freedom of Information and Protection of Privacy Act (FIPPA). For this survey, your identifiable information is collected under section 26 (e) of FIPPA. We will only use the information you share to help improve our Health Navigator team process or to follow up with you directly about your experience. Your responses will be stored on a secure computer server.
  • Only Trans Care BC staff will have access to your responses. We only intend to identify you if you provide your contact information in the final question of the survey. If you do not wish to be identified, please do not share information that might identify you or someone else in your responses to open ended questions.

What are the risks and benefits of participating?
  • We will do our best to protect your privacy. It is possible that staff may be able to identify you from the information you share. You may benefit from participating if you were to contact or use the services provided by the Health Navigator team in the future. Future clients are likely to benefit from improvements to come from this survey.

Can I withdraw from the survey if I change my mind?
  • You may contact Trans Care BC at any time if you want to withdraw your consent. However, it will only be possible to withdraw your consent if: (1) you provide your contact information and (2) we have not yet shared the survey results the Trans Care BC staff.
