  • No names are being collected – your answers will be kept confidential. If you provide any written comments in the survey, please do not include any information that might identify you or someone else. When survey results are reported, presented, or published we will not include any information that could identify you or someone else.
  • The results will be reported in summary form and we will not identify any one person.
  • Survey data (captured via Checkbox) will be stored in Canada.
  • Only the staff at PHSA will access the survey findings.  The staff will run the survey and analyze the results. Survey data will be stored in the PHSA’s secure network.
