You are invited to participate in a nationwide research study conducted by researchers at the British Columbia Centre for Disease Control (BCCDC) in Vancouver. By taking part in the survey, you will be entered into a draw to win one of two $500-dollar gift cards. Before agreeing to participate in the research study, please carefully read the following information.

Study Title: Examining the attitudes, knowledge and clinical practice of Canadian health care providers towards HPV vaccination in men.

Aim of the study:
The survey has been designed to better understand Canadian health care providers’ attitudes, beliefs, knowledge and clinical practice regarding the use of HPV vaccination in men, with a particular focus on men who have sex with men (MSM). The results of this study will be important for clinicians and public health practitioners in informing vaccination policy and practice.

Study Procedures:
  • The survey will take about 15 minutes to complete.
  • The questionnaire is confidential. Please do not include any information other than what is asked.
  • Participation is voluntary.
  • You can leave blank any question that you prefer not to answer
  • You may freely withdraw consent at anytime. Simply close the survey window to delete all your responses and to exit the survey.
  • Once the questionnaire has been completed and submitted, you will no longer be able to withdraw from the study.
  • Upon completion of the survey, you will have the option of opening a separate link to leave your email address, for entry into a draw to win one of two $500-dollar gift cards. Rest assured that your email address will not be linked to your survey response.

Potential Risks:
There are no potential risks associated with participation in this study.

Potential Benefit:
Benefits to health care provider participants may include gaining new knowledge around the use of HPV vaccination in males and MSM populations. Completing the survey also provides an opportunity to be entered into a draw to win one of two gift cards.

All information collected during the study will remain confidential. Completed surveys will be securely stored until completion of the study. After this time, all surveys will be destroyed. Access to study surveys will be limited to the study investigators and their collaborators, the University of British Columbia Research Ethics Board for monitoring purposes, and the survey provider (Checkbox). The results of this study may be presented at conferences or other public forums, and/or published in journals.

Final Results:
For participants interested in the results of this study, there will be an option to provide your email address (via separate link) to receive a report upon study completion.
