Vancouver Coastal Health Accessibility Plan Feedback Form - Patients and Visitors
Your feedback is important in helping us improve accessibility at Vancouver Coastal Health (VCH). We welcome comments on the Accessibility Plan and any recommendations you may have.

Feedback will be collected, analyzed and reviewed by VCH’s Accessibility Committee and Leaders Working Group. It will help to identify priorities and a systems-wide approach to improving accessibility and can be incorporated into future accessibility plans.

If you have a concern or complaint regarding care that you or someone you know received as a person with disability, you can connect with the Patient Care Quality Office.
