As a patient of BC Cancer or a member of the public living in BC or the Yukon, please complete this Information Request Form if you would like to receive customized cancer information from the BC Cancer Library. You will be asked for some contact information, the type of cancer information you are looking for, and how you would like this information

Your Personal Information

Your personal information is protected by our privacy law in BC (the Freedom of Information and Protection of Privacy Act (FIPPA)). If you provide your contact information to us through this Information Request Form, your responses will be stored on PHSA internal servers in compliance with Section 26 (ii A) 32 B-C of FIPPA solely for the purpose of  responding to your information request. Your personal information will be kept confidential, be accessed only by BC Cancer Library staff, and will not be retained after your information request has been completed.  If you have any questions regarding the use of your personal information, please contact PHSA Information Access and Privacy Services at privacyandfoi@phsa.ca.
